New Patient Information and FAQ

  • What Insurances are accepted?

    We are in-network and currently accepting new patients with the following PPO policies:

    Blue Cross and Blue Shield/ Bridgespan: Participating, Preferred, Individual&Family, RealValue only 

    includes the entire BCBS family: Regence, Anthem, etc.

    PacificSource (not OHP Community Solutions)

    Providence (Signature Network, not a PCP for Choice)

    Aetna (including Meritain and GEHA)

    United / UMR

    HMA

    Moda (Connexus and Affinity Networks only)

    Cigna

    We are not "in plan" for health plans that require a "medical home," nor do we participate with HMO plans, Trillium, OHP, or any form of Medicaid.  We cannot accept patients with Medicaid as primary or secondary insurance.  It is against our Medicare contract to accept cash payments from patients who qualify for state or federal medical assistance programs, so we cannot treat you even if you are willing to pay cash.

    Please read your insurance card.  If your card says, "HMO" or "medical home", we are not covered by your plan even though we may be "in network" for that insurance company.  If your card says "PCP required", please call your insurance company to designate this provider as your PCP BEFORE your visit.  We are not accepting patients with OMIP plans.

    It is the patient's responsibility to understand their own health plan, and to verify with their insurance that an office visit in our clinic is covered.  The patient will be fully responsible for charges that insurance does not cover.

  • How do I sign up as a new patient?

    Use the link below to register and schedule if you are:

    *Ages 12 to 59

    *covered by an in-network insurance

    *located in the State of Oregon

    Please refrain from sending a message or calling requesting an exception. These guidelines are set by contractual obligations.

    Register only when you are ready to submit ACTIVE insurance and set a date for your first appointment. Your file will be deleted in three days if we do not receive your insurance information and ID, AND have your new patient appointment scheduled. 

    Example, if your insurance becomes active on January 1, please do not register until January 1 as it will show up as INELIGIBLE until the ACTIVE DATE.

    Existing patients may access their portal and new patients may sign up for the patient portal by clicking right here.

    1) Click the link above and sign up for a patient portal account by clicking “create an account.” Please complete all fields under "Contact Information" in your profile. You may also set your notification preferences. We do not sell your information. Please enable text messaging for best service.

    2) Schedule your first appointment online in the portal at the Schedule Now button.

    3) Click on the message bubble at the top of the page. Compose a new message titled insurance. Attach photos of the front and back of your insurance card and photo id. (Works best from a smart phone.)

    Your first appointment is a New Patient visit to establish care. It will not be billed as your annual physical. Your provider will get to know you and your history, order lab work, and schedule your SECOND appointment as your annual preventive care visit. Your second visit cannot be your physical if treatment is necessary based on lab results or other factors. We ask that all patients schedule a preventive care visit each year.

  • Do you accept Medicare and Medicaid (OHP) Patients?

    We see Medicare patients.  However, we are not accepting new patients with original Medicare, MedAdvantage or Medigap plans.  Also, we are not accepting new patients with Medicare as their secondary insurance.  

    We do not have assignment from Medicaid/OHP.  Due to contract restrictions, we cannot see these patients on a self-pay basis nor bill patients for whom OHP/Medicaid is secondary insurance.

Frequently Asked Questions

What payment methods do you accept for copays and out of pocket expenses?

We accept credit/debit cards at the time of your visit. Financing is not provided by our office. Cash is not accepted. Checks may be mailed to our billing partner upon receipt of an invoice.

Do you process Worker's Comp and/or Motor Vehicle Collision claims?

Our PCPs process motor vehicle claims for our existing patients only. Dr. John accepts new MVA cases for chiropractic care. Our PCPs do not process Worker's Comp claims as that process requires you to see an in network doctor. However, Dr John will treat workers comp cases.

I'm sick.  Can I get in to see you today?

YES!  Same day openings will show in online scheduling until 3:30pm or call the front desk if you cannot find availability.

Do you offer sliding scale or discounted rates?

If you are seeking an opportunity to see our providers at low or no cost, they provide pro bono services in the community, not through this private practice.

Do you provide immunizations?

We do not stock vaccinations in our office.  Our providers educate on recommended preventive immunizations and refer patients to their pharmacy.
